OP Nurse Coordinator I - Comprehensive Lung Center (UPMC Presbyterian) New

UPMC Presbyterian is looking to hire a Full-Time OP Nurse Coordinator I to support the Comprehensive Lung Center.

Schedule:

• 8 am to 4:30 pm
• Monday-Friday
• This role will allow no more than 2 days a week working remotely.
  
  

Purpose:

The Outpatient Nurse Coordinator is a Registered Nurse and member of the care delivery team who is committed to the community and the nursing profession. The RN is responsible for the patient’s care from a global perspective and is accountable for multiple aspects of the patient care package, even when direct patient contact is limited. The role requires independent judgment and ongoing communication with patients, families, physicians, and community resources to support patients in achieving outpatient health.

Responsibilities:

• Demonstrates accountability for professional development to improve the quality of professional practice and patient care. Actively participates in practice-based shared governance, goal setting, and change initiatives to enhance care quality and the practice environment. Serves as a fully engaged care team partner, responding willingly to team needs and contributing to efforts that improve patient care and professional practice. Adapts to change with flexibility and provides oversight and supervision of the practice environment, assisting other disciplines and directing clinical and administrative activities as needed. Demonstrates leadership with nursing staff, patient information coordinators, medical assistants, and office assistants in the daily management of patient processes.
• Formulates patient-centered goals and plans of care that involve patients as partners and address holistic needs. Triages patient situations, coordinates care with physicians, and assists with all aspects of patient care, including assessment, evaluation, and education. Provides continuous, global responsibility for patient care beyond office or procedural visits, ensuring outpatient treatment follow-up and serving as the primary nursing resource for assigned patient caseloads. Facilitates services outside the healthcare facility and collaborates with home care and support services.
• Develops and maintains productive internal and external relationships through accountability, enthusiasm, motivation, and commitment to patients and colleagues. Demonstrates cultural awareness, holds peers accountable for healthy relationships, balances work and personal life, and models safe work hours, effective time management, and healthy lifestyle practices. Communicates workplace safety hazards and assists departmental management with patient care planning, operations, and innovations.
• Actively participates in practice- or department-specific quality improvement initiatives, identifies opportunities for improvement, and takes personal responsibility for enhancing patient satisfaction. Uses research and evidence-based practice to improve clinical care by identifying relevant research issues and literature. Assists with regulatory agency reviews and quality audits and serves as a patient advocate to physicians and physician extenders.
• Demonstrates the ability to establish positive, caring relationships across all organizational levels and with patients and families, function effectively in complex environments with changing priorities and specialized equipment, and apply strong clinical judgment, critical thinking, analytical, and problem-solving skills. Exhibits communication skills that position the nurse as the patient’s advocate. Meets physical requirements including mobility, manual dexterity, visual acuity, and stamina for walking, standing, lifting, and positioning patients.
• Applies critical thinking to identify clinical, social, safety, psychological, and spiritual patient needs within an episode of care. Communicates clearly to articulate patient conditions, negotiate care changes, and recommend practice improvements. Maintains comprehensive documentation that supports caregiver communication and aligns with national professional organizations, business unit, and health system goals to improve safety, quality, and satisfaction. Creates a compassionate, patient-focused experience by building healing relationships.
• Demonstrates knowledge of adult learning principles (and pediatric teaching when applicable) and applies them in educating patients, families, students, and staff through in-person, phone, and written communication. Serves as the primary nursing liaison for total care planning, supports the development of students and colleagues, may function as a preceptor, coordinate staff education and training, and serve in a senior nursing role to foster professional growth. Promotes an environment of open dialogue, inquiry, and continuous improvement through feedback and practice refinement.
• Minimum 3 years nursing experience required.
• BSN preferred.
• Outpatient nursing experience preferred.
  
  

The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.

Licensure, Certifications, and Clearances:

Current licensure as a Registered Professional Nurse in practicing state. CPR Certification. UPMC approved national certification preferred. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
